COUNTY OF ST. LUCIE
BEFORE ME, the undersigned authority, personally appeared
JERRY VAN BLARICOM who, being duly sworn, says that he is the
Lienor herein, or the authorized agent for the lienor, J & J
PLUMBING SERVICE, whose address is Post Office Box 3427, Ft.
Pierce, Florida, and that pursuant to a contract with Les
Reeley Construction Co. and James F. Harmon, B. J. Herndon
a/k/a Billy Herndon, and Clifton Lloyd, Jr. a/k%a Clifton
Lloyd, as Trustees and constituting the local Board of Trustees
of the Palm Vista Church of God, formerly known as The Church
of God at Ft. Pierce, St. Lucie County, Florida. Lienor fur-
nished labor, services or materials consisting of plumbing
labor, services and supplies on the following described pro-
perty in St. Lucie County, Florida:
The South 1/2 of the SW 1/4 of the SW 1/4 of the SE 1/4
of Section 8, Township 35 South, Range 40 East;
owned by James F. Harmon, B. J. Herndon a/k/a Billy Herndon
and Clifton Lloyd, Jr. a/k/a Clifton Lloyd, as Trustees and
constituting the local Board of Trustees of the Palm Vista
Church of God, for a total value of $3,635.00, of which there
remains unpaid $3,444.50, and furnished the first of same on
November 4, 1978, and the last of same on May 25, 1979.
