'y THIS AGREEMENT, made and entered into this ~ day of
, 19~, by and between FELIPE P. LADIA, l[.D.,
P.A. (herein referred to as the "Employer") and FELIPE P. LADIA,-
and LILIA LADIA (herein referred to as the Trustee").
W I T N E S S E T H:
WHEREAS, the Employer hereby establishes a pension plan
and trust effective Msy 1 , 1976, known as the FELIPE P. LADIA,
M.D., P.A. PENSION PLAN AND TRUST (herein referred to as the
"Plan") in recognition of the contribution made to its successful
operation by its employees and for the e=clusive benefit of
its eligible employees; and
WHEREAS, under the terms of the Plan, the Employer has
the ability to amend the Plan, provided the Trustee 3oins
in such amendment if the provisions of the Plan affecting
the Trustee are amended; and
NOW, THEREFORE, effective May 1, 1976, the Employer and

the Trustee, in accordance with -the provisions of the Plan pertaining
to qualification hereby state the Plan to proof de as followd
ARTICLE I - DEFINITIONS
~ 1.1 "Accrued Benefit" shall be the sum of:
(a) a Participant's Accrued Benefit as of the
~ Anniversary Date of the Plan immediately prior to the
1 Effective Date of this Amendment, plus
'i (b)' the retirement benefit a Participant would
receive at his Normal Retirement Date as provided
in Section 5.1(a) of this Agreement, less (a) above,
multiplied by a fraction, not greater than one (1), the
numerator of which is the Participant's total number
of Plan Years of Serviee commencing with the Effective
