PETITION FOR APPOINTMENT OF GUARDIAN
The undersigned petitioner alleges:-
1. Petitioner's residence is Route 5, Box :222, Fort Pierce,.
Florida, and petitioner's post office address is Suite 204,
Citizens Federal Building, 8410 South Federal Highway, Port
St. Lucie, Florida.
2. HELENE GRAF is an incompetent, whose date of birth
was December 30, 1899, who is 79 yeas of age, and whose social
security number is 299-28-0719. The residence of the incompetent.
is 207 North 17th Street, Fort Pierce, Florida, and the post office
address of the incompetent is the same.
3. The nature of the incompetent's incapacity is senility,
as adjudged by the .Circuit Court in Fort Pierce, St. Lucie County,
Florida, on June 1, 1979.
4. It is necessary that a guardian of the person and property
of HELENE GRAF be appointed for the incompetent.
S. The approximate value and description of the incompetent's
property is approximately $12,S00.00, the exact amount unknown to
petitioner, which property is cash in the possession of
WARD J. GRAF, 2242 Belvedere Street, Port St. Lucie, Florida.
