BEFORE ME, the undersigned authority, personally appeared
HAZEL HOLLY, formerly known as HAZEL ANTONIDES, who upon first being duly sworn,
deposes and says
1. MY name is HAZEL HOLLY and I was married to WILLIAM R. HOLLY
on June 26, 1976 at West Hollywood, Broward Coun y, or a. .
2. The Affiant and husband now reside at 8761 Johnson Street,
Pembroke Pines, Broward County, Florida.
3. Prior to npr present marriage, Affiant was married to RAYMOND L.
ANTONIDES on October 30 1926 at Elkton, Mar land,

without the intervention of any divorce proceedings, from that date until the
date of decedent's death on May 23, 1975
4. That the property situate, lying and being in St. .Lucie County,
Florida, and legally described as follows:
Lot 16, Block 1457 of PORT ST. LUCIE SECTION 15,
a Subdivision according to the Plat thereof,
as recorded in Plat Book 13 at page 6 of the
Public Records of St. Lucie County, Florida.
was purchased by the Affiant and decedent spouse on Au ust 29 1974
as tenants by the entireties by Deed dated the 29th day of ugust, 1974, and
recorded on the 2nd day of October, 1974 in Official Records Book 232 at page
1478, Public Records of St. Lucie County, Florida.
5. FURTHER AFFIANT SAYETH NOT.
