The undersgned, as personal representative of the above estate alleges: w
1. The decedent, Anna I)e Piazza a/~[~ a ~ti Pia~~a a resident of

October 26 19 75_ ,and Letters of Administration were issued to

2. Petitioner files herewith a flnai accounting containing a rnmplete report of all receipts
and disbursements since the commencement of administration of this estate, or since the date of the
last accounting f~7ed herein, if any.
3. Petitioner has fully administered thus estate, by making payment, settlement, or other
disposition of aII claims and debts that were presented, and by paying or making provision for the
payment of all expenses of administration.
4. Petitioner has fled all required estate tax returns with the Internal Revenue Service and
.with the Department of Revenue of the State of Florida, and has obtained and filed with this court
evidence of the satisfaction of this estate's obligations for both federal and Florida estate taxes, if any.
5. Petitioner has made or proposes to make distn'bution of the assets of this estate as set
forth in the schedule of distnl~ution attached hereto as Exhibit A.
