The undersigned petitioner alleges:

2. IVY JULIA RERENS is an incompetent, whose date of birth
was July 24, 1894, who is 85 years of age, and whose social security
number is 571-72-6858. The residence of the incompetent is
367 S.E. Tranquilla Avenue, Port St. Lucie, Florida 33452 and the
post office address of the incompetent is the same.
3. The nature of the incompetent's incapacity is senility,
as ad3udged by .the court in the Circuit Court for the Nineteenth
