. THIS CAUSE having core oa for hearing on the petition of the DSPARTlIEITt
OF HEALTH AND REEiASiLITATIVB SERVICES for child support and the Court having found
that said Petitioner is a proper party by virtue of an assig~ent of rights to
child support signed by the party having custody of the dependent child(ren) sad
all parties baying received proper notice, it is
ORDERED AND ADJUDGED that the Petition for Child Support shall be sad
it is hereby granted; it is further
ORDBRBD-AND ADJUDGED-that coswcncing on ~ Z. , 1979,
the responsible parent LINDA MAE GARDR ,who is the mother
of the sinor child(ren), to-wit: CAROLYN GARDNER, d . o . b . 1-14-71

Said amount shall be reaitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Bnforcement Unit, 1317 Winewood Bouleroard,
Tallahassee, Plorida, 32304. It is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and (s)he
is hereby ordered to continue to transmit support payments received from
the Reapoadent
to the Department of Health and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notice to Discontinue Payments Eros the Departsent
of Health and Rehabilitative Services, in which event the support payments shall there-
after be directed and payable to the person having custody of the child(.ren).
