is/are declared to be the legitimate child ren) of the Defendant

the natural .mother; it is further
ORDERED AMID ADJUDGED that the natural mother,
MARVARETTA STOCKTON ,shall have custody of the said child(ren)
subject to the Defendant s right of reasonable visitation; it is further

Said amount shall be remitted monthly by the Clerk to the Department of Health and.
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304; it is further - -
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and is
hereby ordered tv continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payment$ from
the Department of Aealth and Rehabilitative Services, fn which event the support
payments shall thereafter be directed and payable to the aforesaid natural mother
or person having custody of the. child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTEi AND REHABILITA?IVE
SERVICES, BUREAU OF VITAL STATISTICS, AHENpMENT UNIT, shall and it is hereby ordered to:
