IBIS CAUSB having come on for hearing on Intervenor's Petition
for Modification of final judgsent or order on child support and the CourC
having reviewed the file and considered the testimony of witnesses and being
otherwise fully advised in the presdses, it is -
ORDBRED AND ADJUDGED that the Petition for lodification of final
judgsent or order on child-support shall be and the same is hereby granted;
it is further
ORDERED AND ADJUDGED that commencing on S L - ,
19 79 the Father, JOHNNIE PEEPLES shall pay to the
Clerk of this Court .the sub of $ 35.00 per week each and ever
week plus $2.00 statutory fee, which amount shall be reaitted
monthly by the Clerk to the Department of Health and Rehabilitative Services,
Child Support Enforcement Unit, 1317 i~Iinewood Boulevard, Tallahassee, Florida,
32304. .All payments shall be made in cash, money order or cashiers check. All
money orders and cashiers checks shall bear the payees name and Social Security

it is further
ORDERED AND ADJUDGED that in addition to the above-ordered amounts
the sum of $ per shall be and the same is hereby
ordered to be paid as set forth herein as and for arrearages or past unpaid
child support which is due and owing in a total amount of $ .
Said arrearage payment shall continue until entire arrearage is fully paid;
it is further
- ORDERED AND ADJUDGED that the~Clerk of this Court shall and he is
hereby ordered to continue to transmit support payments received from the
responsible parent to the Department of Health and Rehabilitative Services until
further order of this Court to the contrary or receipt of Notice to Discontinue
Payments from t'he Department of Health and Rehabilitative Services, in which
event the support payments shall thereafter be directed and payable to the person
having custody of `the child(ren).
