BEFORE ME, the undersigned authority, personally appeared, -
LEONARD SIMONS, who after being first duly sworn upon oath deposes
and says: -
1. That~he is the owner of the following described pro-
erty, to-wit:
Lot 6, Block 496, PORT ST. LUCIE SECTION TEN,
according to the-plat thereof, as recorded in

the Public Records of St. Lucie County, Florida. -
2. That he had contracted with BURPHY CONSTRUCTION, INC.,
to be the general contractor for the construction of the home located
on the above described property; that said corporation is a aegis--
- tered corporation to general contract and has performed all general
contracting duties.
3. BURPHY CONSTRUCTION, INC., has sub-contracted portions

undersigned because of construction upon the subject property have

in the office of the Clerk of the Circuit Court, and a copy of this
Affidavit has been served on each of the lienors, or possible lienors,
listed above. -
6. That the owner did not have privy to contract with the
sub-contractor who has defaulted and abandoned the project, but intends #
to authorize the general contractor to recommence those portions of
' the sub-contractor's work. -
