The undersigned petitioner alleges:
1. Petitioner's residence is 8028 South Federal Highway, Port St.
- Lucie, Florida, 33452, and petitioner's post office address is 8028 South Fed-
- eral Highway, Part St, J.ucie, Florida, 33452: - _ -
2. DORIS~. O'SULLIVAN is an incompetent, whose date of birth was -
" June 29, 1916, who~is 63 years of age, and whose social security number is
050-14-9123. The residence of the incompetent is 2050 Triumph Road, Port
St. Lucie, Florida 33452, and the post office address of the incompetent is
2050 Triumph Road, Port St. Lucie, Florida, 33452.
3. The nature of the incompetent's incapacity is that she is suffering
frcnr. Ironic brain syndrome which condition tends Eo confuse the incompetent
as to time and place. She is also suffering from mild physical incapacity in
" that she would require a moderate amount of physical assistance to get around.
4. It is necessary that a guardian of the person and property be ap-
pointed for the incompetent.
5. The approximate value and description of the incompetent's proper-
ty are:
A. Savings Account with First Federal Savings and Loan Association
of Martin County, Jensen Beach Office in the amount of $18, 318.32.
B. Savings Account with First National Bank and Trust Company of
Stuart in the amount of $200.00.
C. Checking Account with First National Bank and Trust Company of
Stuart in the amount of $4, 400.00.
