THIS CAUSE having core on for hearing on the petition of the DEPARI?!IIR
OF HEALTH AND RBEiABILITATIVB SERVICES for child support and the Court having found -
that said Petitioner is a proper party by virtue of an assigcmzent of rights to -
child support signed by the party having custody of the dependent child(ren)-and
all parties having received proper notice, it is _
ORDERED AND ADJUDGED that the Petition for Child Support shall be sad
it is hereby granted; it is further -
ORDERED AND ADJUDGBD that cosmencing on _ September 21 , 1979, -

Said amount shall be resitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enfo~ccement Unit, 1317 WineWOOd Boulevard,
Tallahassee, Florida, 32304. It is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and (s)he
is hereby ordered to continue to tranesit support payments received Eros the Respondent
to the Department of Health and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notice to Discontinue Payments fros the Department
of Health and Rehabilitative Services, in which event the support payments shall there-
after be directed and payable to the person having custody of the child(ren).
