1gIS CAUSE having come on for hearing on the petition of the DEPARI!lBNT -
OF HEALTH AND REHABILITATIVE SERVICES for child support sad the Court having found
that said Petitioner is a proper party by virtue. of an assign*ent of rights to
child support signed by the party having custody of the dependent child(ren) and
all parties having received proper notice, it is
ORDERED AND ADJUDGBD that the Petition for Child Support shall be and
it is hereby granted; it is further
ORDERID AND ADJUDGED that cosiiaencing on =September 21 1979,
the responsible parent BETTY DENNARD ,Who is the mother ,
of the minor child (rem) , to-vit : ANTHONY GREG WILLIAMS , d . o . b . 6 -11- 67 ;
SONYA LaSHON TUCKER, d.o.b. 12-23-71 -
shall pay to~ the Clerk of this Court the artw of $12.00 -per week each and
every week ,plus 52.00 statutory-fee. All payments shall be made in cash, -
money order or cashiers check. All money orders and cashiers-checks shall bear the
payees Heise and Social Security Nuvber and shall be made payable to the CLERK OF THE _
CIRCUIT COURT, acrd sent to:

Support Department

Fort Pierce, Florida 33450.
Said amount shall be remitted monthl~r by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewod Boulevard,
Tallahassee, Florida,-32304. It is further _
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and (s)he
is hereby ordered to continue to transmit support payments received from the Respondent
_to the Department of Health and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notice to Discontinue Payments from the Departiieat
of Health and Rehabilitative Services, in riid?ich event the support payments shall there-
after be directed and payable to the person having custody of the child(ren).
