THIS CAUSE having come on for hearing and all_parties having received
proper and timely notice; the Court having heard-testimony and/or considered the
pleadings, papers, affidavits and other papers filed herein, and being otherwise
fully and yell advised in the premises, it is
ORDERED AND ADJUDGED that the minor child(ren) -

MARIO STEPHON GREENS, d.o.b. 8-22-70
is/are declared to be the legitimate child(ren) of the Defendant
LLOYD COBB ,and VIRGINIA GREENS ,
the natural mother; it is further _
- .ORDERED AND ADJUDGED that the natural mother. '
VIRGINIA GREENS ,shall have custody of the said child(ren)
subject to the Defendant's right of reasonable .visitation; it is further ~
ORDERED AND ADJUDGED that commencing on October 1 , 1979,
'i the Defendant/Father shall pay child support for and on behalf of the said child(ren)
in the amount of $ 20.00 per week • ,plus $2.00 statutory -fee. All
payments shall be wade in cash, money order or cashiers check. -All money orders and =
cashiers checks shall bear the payees name and Social .Security Number and shall be

Said amount shall be remitted monthly by the Clerk to the Department of Health-and
Rehabilitative Services, Child Support Enforcement Unit, 1317 WineMOOd Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and is
hereby ordered to continue to transsit support payments received frow the Defendant
until further order of this Court or receipt of Notice to Discontinue Paywents from
the Department of Health and Rehabilitative Services, in ~rhich event the support
paywerits shall thereafter be directed and payable to the aforesaid natural sother
or person having custody of the child(ren); it is further
- ORDERED AND ADJUDGED that the above-nailed Defendant having been adjudicated ~
the father of the above-named child(ren), the DEPARITlENT OF HEALTH AND REAABILITATIYE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to:}

1. amend the above-named child's/children's bir-th certificate(s) to shoo the
above-named father's name.
2. resove from the above-naiaed child's/
children's birth certificate(s) and enter the above-Hawed father's name.
