Before me, the undersigned authority, personally appeared Doris
P. Day, who, upon first being duly svrorn, did depose and say:
That her name is Doris P. Day and she has resided at 7005
Santa Rosa Blvd., Ft. Pierce, Florida for three years.
Affiant further states that before taking up residence in
St. Lucie County shy was a resident of Dayton Trailer Court,
Hobe Sound, Florida in Martin County.

lien filed against Doris Day of 620-B North Dundas Court, Fort

~ Pierce, Florida by Fort Pierce Memorial Hospital on November 21,
1975 recorded in O. R. Book 246 at Pa a 78.

Affiant further states that she is not the person named in that
particular lien and has never been a patient in Fort Pierce
~ Memorial Hospital and has never used a Dundas Court mailing
address in Fort Pierce, Florida in all of her life.
