This cause was duly presented by the State Attorney and the Respondent having been present in
Court, and the Court being advised in the premises, it is,

ORDERED AND ADJUDGED that the Respondent pay to the Petitioner for the benefit of
JAMIE ADKINS, born 9/14/69

minor children, the sum of S 250.00 pei month plus and additional l $2.00
for payment of Clerk's fee for receiving and disbursing support payments.
These payments are to begin on the
first day of November 19 7 9 ,and thereafter on first
of each and every month that said payments shall be made. in cash, by cashier's check

or by postal money order to the Clerk of the Circuit Co~rt of St. Lucie County,
P. O. Box 700, Fort Pierce, Flori a, ~
Florida and the said Glerk shall transmit the said payments to Child Support Enforcement '
Section, 1317 Winewood Boulevard Tallahassee, F on a 3Z or rans-
mittal to the State of West Vir inia as long as the case is certified as
a m;ttP Tv-n c-aae Tf the Cler receives notification by the Department
of Health & Rehabilitative Services that the case is no longer a 1'V-D case,
he will then forward all support to: Clerk of Circuit Court of Kanawha County,;
P O Box 235. Charleston, West Virginia 25328. (Include name. of Petitioner
and Respondent on ALL CHECKS.)
That this Court shall retain jurisdiction of this cause for the purpose of enforcing this Order
and entering such further orders as may be deemed equitable.
