PETITION FOR APPOINTMENT OF GUARDIAN
The undersigned Petitioner alleges:
1. Petitioner's residence is 1701-- 44th Avenue,Vero Beach,
Florida, and Petitioner's post office address is Post Office Box
100, Vero Beach, Florida 32960.
2. MICHAEL ALAN NOONAN is a minor, whose date of birth was
September 26, 1974, who is four and one-half years of age, and

whose Social Security Number is 266-77-8460, The minor is in '
the custody of the State of Florida, Department of Health and
i
Rehabilitative Services, located at 201 South Indian River Drive,

Fort Pierce, Florida 33450, and the post office address of the
minor is Adoption and Related Services, 201 South Indian River
Drive, Fort Pierce, Florida 33450.

3. The nature of the incapacity of MICHAEL ALAN NOONAN is
minority.
4. It is necessary that a guardian of the property be
appointed for the minor.
5. The approximate value and description of the minor's
property are:

Approximately $2,929.82 cash, to be received by MICHAEL ALAN
NOONAN as a joint beneficiary of an inter vivos trust created by #
GRACE SANDERS, deceased, and presently held by Indian River #
Federal Savings and Loan Association in Savings Account X30245 in
the names of RICKY CHARLES LANE and MICHAEL ALLEN NOONAN as
joint beneficiaries of the GRACE SANDERS TRUST, with present '
balance of $5,859.63.
6. The minor is rese t
p n ly in the custody of the State of ,
Florida, Department of Health and Rehabilitative Services which

pursuant to Florida Statute 63.052(2) is the official guardian
of the said minor which has been placed for adoption with, and
permanently committed to, said department.
