the Department of Health and Rehabilitative Services for leave to
intervene in this case and the Court having found tt~ t all parties
were properly put on notice and having heard arguments presente3
and considered the evidence and testimony; the Court Finds that

Rehabilitative Services shall be and it is hereby substituted for
the mother of the minor child(ren) of this marriage in all matters
