PETITION FOR SUMMARY ADMINISTRATION
The undersigned Petitioner alleges:
1. Petitioner has an interest in the above Estate as the
surviving spouse and sole beneficiary named in the decedent's Iasi
Will. Petitioner's name and address are as set forth in Paragraph 3,
and the name and address of Petitioner's attorney are as set forth at
the end of this Petition.
2. Decedent, JOE ALLEN, whose last known address was 4531
Central Avenue, Middletown, Ohio 45042, whose age was 62 years, and
whose Social Security Number was 273-16-9697, died on July 25, 1978,
at Cincinnati, Ohio, and at the time of death decedent was domiciled -
in Butler County, Ohio.
3. So far as is known, the names of the beneficiaries of this
Estate, their addresses and relationships to decedent, and the ages of
any who are minors, are:
OPAL ALLEN, surviving spouse, over 18 years of age;
4531 Central Avenue
Middletown, Ohio 45042
4. Venue of this proceeding is in this county because the
decedent owned real property in St. Lucie County, Florida.
S. The assets in this Estate and their estimated values are
as follows
Lots 4 and 5, Block 146, LAKEWOOD PARK
UNIT 11, according to the Plat thereof
on file in Plat Book 11, pages 32A-32D,
of the Public Records of St. Lucie
County, Florida. $5,000.00
6. The value of the entire Estate subject to administration
in this State does not exceed $10,000.00.
