PETITION FOR DISCHARGE OF GUARDIAN
The undersigned, as Guardian of the person and property of
DAVID KEITH HODGES, a minor, alleges:
1.. The reed for the continuation of this Guardianship no
longer exists because the minor reached the age of eighteen (18) years
on October 5, 1979.
2. Petitioner files herewith a final return containing a full
and correct account of all receipts and disbursements of all the
ward's property of which Petitioner has control since the last return
filed herein.
3. Petitioner represents that DAVID KEITH HODGES, now an
adult, files herewith his written waiver and consent to the entry of
an Order discharging Petitioner as Guardian.
WHEREFORE, Petitioner requests that an Order be entered

approving all accountings not previously approved and discharging
~ Petitioner as Guardian and releasing the surety on Petitioner's bond
from further liability.
Under penalties of perjury, I declare that I have-read
the foregoing and the facts alleged are true, to the best of my
~ knowledge and belief.
