THIS CAUSE having cove on for hearing and all parties having received
proper and timely notice; the Court having heard testimony and/or considered the
pleadings, papers, affidavits and other papers filed herein, and being otherwise
fully and ~?ell advised in the premises, it is
ORDERID AND ADJUDGID that the minor child(ren)
LACRYSTAL'DUPREE, d.o.b. 11-10-76
is/are declared to be the legitimate child(ren) of the Defendant
WESLEY ALEXANDER SANDS and LENORA FAY DUPREE
the natural mother; it is further
ORDERED AND ADJUDGED that the natural mother.
LENORA FAY DUPREE shall have custody of the said child(ren)
subject to the Defendant's right of reasonable visitati/on~; it is further
ORDERID AND ADJUDGID that commencing on /Vv 1/ 2- ~ . 1979,
the Defendant/Father shall pay child support-for and on behalf of the said child(ren)
in the amount of $ ~ O C~ per ~.cJ e ~Q , plus $2.00 statutory fee. All
I! payments shall be made in cash, money order or cashiers check. All money orders and
cashiers checks shall bear the payees Howe and Social Security Number and shall be
made payable to the CLERK OF THE CIRCUIT COURT, and sent to:

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Wine rood Boulevard,
~ Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGID that the Clerk of the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments from
the Department of Health and Rehabilitative Services, in ~+hich event the support
payment's shall thereafter be directed and payable to the aforesaid natural mother
or person having custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILI?ATIVE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to:
