and all parties having received proper and timely notice; the Court having heard
testitwny and/or considered the pleadings, papers, affidavits and other papers
filed herein, and being otherwise fully and well advised in the premises, it is
ORDERED AND ADJUDGED that the minor child(ren)
TIFFANY ZIESHA MONIQUE COLLINS, d.o.b. 7-14-79
is/are declared to be the legitimate child(ren) of the Defendant
HARRY LEE GERMAN , and ADRINNA COLLINS ,
the natural mother; it is further
ORDERED AND ADJUDGED that the natural mother,
ADRINNA COLLINS shall have custody of the said child(ren)
subject to the Defendant s right of reasonable visitati~ont; it is further

the Defendant/Father shall pay child support for and on behalf of the said child(ren)

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard, t
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and is
s hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payoents from
the Department of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural mother or
r person-having custody of the child(ren); it is further
q ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to:;
