THIS CAUSE having come on for trial upon the pleadings filed herein
and all parties having received proper and tersely notice; the Court having heard
testimony and/or considered the pleadings, papers, affidavits and other papers
filed herein, and being otherwise fully and well advised in the premises, it is.
ORDERED AND ADJUDGED that the minor child(ren)
PHILLIP LEE SYLVESTER, d.o.b. 3-26-74; PAULINE JEANETTE SYLVESTER, d.o.b.
12-12-72
is/are declared to be the legitimate child(ren) of the Defendant
JIMMY LEE STAFFORD ROBERTA SYLVESTER ~
and ,
the natural mother; it is further
ORDERED AND ADJUDGED tl?at the natural mother, -
ROBERTA SYLVESTER shall have custody of the said child(ren)
subject to the Defendant s right of reasonable visitation; it is further
ORDERED AND ADJUDCF.D that commencing on December 17 , 1979,
the Defendant/Father shall pay child support for and on behalf of the said child(ren)
in the amount of $ 120.00 per month plus $2.00 statutory fee
All payment shall be made in cash, money order or cashiers check. All monev orders •
and. cashiers checks shall bear the payees name and Social Security Number and shall
be made payable to the CLERK OF THE CIRCUIT COURT, and sent,t

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winevood Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and is
1 - hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments from
the Department of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural mother or
person-leaving custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES BUREAU OF VITAL STATISTICS AMENDMENT UNIT shall and it is hereb ordered to:
