THIS CAUSB having cone on for trial upon the pleadings filed herein
and all parties having received proper and timely notice; the Court having heard
testiaany and/or considered the pleadings, papers, affidavits and other papers
filed herein, and being other~riae fully and Drell advised in the prewises, it is
ORDERED AND ADJUDGED that the winor child(ren)
CURTIS SIMS, JR., d.o.b. 2-3-79
is/are declared to be the legitimate child(ren) of the Defendant
CURTIS SIMS ~ and EARLENE WATSON ~
the natural mother; it is further
ORDERED AND ADJUDGED that the natural mother,
EARLENE WATSON shall have custody of the said child(ren)
subject to the Defendant s right of reasonable visitation; it is further
ORDERID AND ADJUDGED that comwencing on T/ V`d ~ ~ 1979,
the Defendant/Father shall pay child support for and on behalf of the said child(ren)

All payment shall be made in cash, money order or cashiers check. All monev orders

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winerrood Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that the Clerk ~f the Circuit Court shall and is
hereby ordered to continue to transact support payments received fros the Defendant
until further order of this Court or receipt of Notice to Discontinue Payaents from
the Department of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural aother or
person-having custody of the child(ren); it is further
~ ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
- the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES. BUREAU:OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to:

X 1. amend the above-named child's/children's birth certificate(s) to shoe the
above-named father's name.
- 2. remove from the above-named child's
children s birth certificate(s) and enter the above-named father's name.
