and all parties having received proper and timely notice; the Court having heard

filed herein, and being otherwise fully a~ well advised in the premises, it is
ORDERED AND ADJUDGED that the minor child(ren)
NICOLE SHALANDA COLEMAN, d.o.b. 11-8-78
is are declared to be the legitimate child(ren) of the Defendant
COMMIE KNOWLES a/k/a LEONARD KNOWLES , and MARY LOU COLEMAN ~
the natural_mother; it is further
ORDERID AND ADJUDGED that the natural mother,
MARY LOU COLEMAN shall have custody of the said child(ren)
subject to the Defendants right of reasonable visitation; it is further
ORDERED AND ADJUDGED that commencing.on N d U 7-- ~ , 1979,
the Defendant/Father shall pay child support for-and on behalf of the said child(ren)

All payment shall be made in cash, money order or cashiers check. All money orders

Said amount shall be remitted awnthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winerrood Boulevard,
Tallahassee, Florida, 32304; it is further
~ ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments from
the Department of Health and Rehabilitative Services, in which event .the support
~ payments shall thereafter be directed and payable to the aforesaid natural sother or
person•having custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
~ the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to:'

2. remove from the above-named child's
