.THIS CAUSE having come on for hearing on the petition of the DEPARTMENT
OF HEALTH AND REHABILITATIVE SERVICES for child support and the Court having found
that said Petitioner is a proper party by virtue of an assign~ent of rights to
child support signed by the party having custody of the dependent child(ren) and
all parties having received proper notice, it is
ORDERED AND ADJUDGED that the Petition for Child Support shall be and
it is hereby granted; it is further L r -
ORDERED AND ADJUDGED that commencing on /V d ~ ~ ~ 1979,
the responsible parent POLLY HOOPER Who is the mother ~
of the minor child(ren), to-Wit: SHARON HOOPER, d.o.b. 3-22-68;

Said amount shall be remitted monthly by the Clerk to the Department ofQHealth and
Rehabilitative Services, Ch~.ld Support Enforcement Unit, 1317 Winewood Boulevard, .
~ Tallahassee, Florida, 32304. It is further
~ ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and (s)he
is hereby ordered to continue to transact support payments received fros the Respondent
to the Department of Health and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notice to Discontinue Payaenta from the Depart>Aent
t of Health and Rehabilitative Services, in which event the support payments shall there-
after be directed and payable to the person having custody of the child(ren).
