THIS CAUSE having come on for trial upon the pleadings filed herein
and all parties having received proper and timely notice; the Court having heard
testimony and/or considered the pleadings, papers, affidavits and other papers
filed herein, and being otherwise fully and well advised in the premises, it is
ORDERED AND ADJUDGED that the minor child(ren)
ERVIN PERNAL CARSWELL, d.o.b. 10-27-75
is/are declared to be the legitimate child(ren) of the Defendant
EDWARD PERNELL CARSWELL , and CAROLYN KING

the natural mother; it is further i
ORDERID AND ADJUDGED that the natural mother,
CAROLYN KING shall have custody of the said child(ren)
subject to the Defendant s right of reasonable visitation; 1t is further
ORDERED AND ADJUDGED that commencing on November 30 , 1979,
the Defendant/Father small pay child support for and on behalf of the said child(ren)
in the amount of $ 40.00 pe= .month plus $2.00 statutory fee
until the AFDC arrears of $5045.40 are paid in full
All payment Qhall be made in cash, money order or cashiers check. All monev orders
and cashiers checks shall bear the payees name and Social Security Number and shall
be made payable to the CLERK OF THE CIRCI;IT COURT, and sent to:

Said amount shall be remitted monthly by the Clerk to the Department ~ Health and
Rehabilitative.Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and is
hereby ordered to continue to transact support payments received Eros the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments frog
the Department of Health and Rehabilitative Services, in which event the support
~ payments shall thereafter be directed and payable to the aforesaid natural mother or
person-having custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered. to
