THIS CAUSE having cone on for trial upon the pleadings filed herein
and all parties having received proper and timely notice; the Court having heard
testimony and/or considered the pleadings, papers, affidavits and other papers
filed herein, and being otherwise fully and well advised in the preaises, it is
ORDERED AND ADJUDGBD that the winor child(ren)
TEKESIA SHANTRELLE CAMPBELL, d.o.b. 5-27-73
is are declared to be the legitimate child(ren) of the Defendant
JOHNNY B. CAMPBELL • and DEBORAH ROBINSON •
the natural mother; it is further
ORDERID AND ADJUDGED that the natural mother,
DEBORAH ROBINSON , shall have custody of the said child(ren)
subject to the Defendant s right of reasonable visitation; it is further
ORDERED AND ADJUDGED that coriwencing on V Z 3 , 1979, -
the Defendant/Father shall pay child support for and on behalf of the said child(ren)
in the amount of $ 20.00 per week plus $2.00 statutory fee
All payment shall be made in cash, money order or cashiers check. All money orders
and cashiers checks shall bear the payees name and Social Security Number and shall
_ be made payable to the CLERK OF THE CIRCUIT COURT, and sent to:

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 41ineWOOd Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and is
hereby ordered to continue to transact support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payaents from
the Department of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural aother or
person•having custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARIMENT~OF HEALTH AND REHABILITATIVE.
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to:':

above-named father's name. `
2. remove from the above-named child's
children's birth certificate(s) and enter the above-named father's name. -
