by me first duly sworn, says:
1. That she was married to TURNER COATS, SR., who is now
deceased;
2. That during his lifetime he was known as TURNER COATS,
J. TURNER COATS, AND TURNER COATS, SR., and at various times filed
documents utilizing the above names;
3. That she knows of her own personal knowledge that all the
above are the same person.
