THIS CAUSE having come on for hearing on the petition of the DEPARTMENT
OF HEALTH AND REHABILITATIVE SERVICES for child support and the Court having found
that said Petitioner is a proper party by virtue of an assignment of rights to
child support signed by the party having custody of the dependent child(ren) and
all parties having received proper notice, it is
ORDERED AND ADJUDGED that the Petition for Child Support shall be and
it~is hereby granted; it is further
ORDERED AND ADJUDGED that commencing on November 23 , 1979,
the responsible parent OTIS S . GAY , who is the father ,
of the minor child (ren) , to-wit : OTIS LAVELLE GAY , d . o . b . 7 -31- 7 8 ;
T~ATONIA TRONDA rAY, d.o.b. 7-31-79
shall pay to the Clerk of this Court the sum of $ 1 0.00 peT week each and
Ii every week , plus $2.00 statutory .fee,
~ All payments shall be made in cash,
i money order or cashiers c ec 1 money orders and cashiers checks shall bear the
I~ payees name and Social Security Number and shall be made payable to the CLERK OF THE
f CIRCUIT COURT, and sent to:
Clerk of the Circuit Court
Support Department

Fort Pierce, Florida 33450.
Said amount shall be remitted monthly by the Clerk to the Department of Health and
:Rehabilitative Services, Child Support Enforcement Unit, 1317 Hinewood Boulevard,
Tallahassee, Florida, 32304. It is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and (s)he
is hereby ordered to continue to transmit support payments received from the Respondent
to the Department of Health and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notice to Discontinue Payments from the Department
of Health and Rehabilitative Services, in which event the support payments shall there-
after be directed and payable to the person having custody of the child(ren).
