THIS. CAUSE having coae on for trial upon the pleadings filed herein
and all parties having received proper and timely notice; the Court having heard
.testimony and/or considered the pleadings, papers, affidavits and other papers
filed herein, and being otherwise fully and yell advised in the premises, it is
ORDERED AND ADJUDGED that the minor child(ren)
TARUS TERRELL MOORS, d.o.b. 5-18-75
is/are declared to be the legitimate child(ren) of the Defendant
J. W. HILL, JR. and CHERYL MOORS
the natural mother; it is further _
ORDERED AND ADJUDGED that the natural mother,
CHERYL MOORS shall Dave custody of the said child(ren)
subject to the Defendant s right of reasonable visitation; it is further
ORDERED AND ADJUDGED that commencing on C ~ ,-1979,
the Defendant/Father shall pay child support for and on behalf of the said child(ren)

All payment shall be made in cash, money order or cashiers check. All money orders

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Wineweod Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that the Clerk ~f the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments froa
the Department of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural mother or
person -having custcxly of the child(ren); it is f~~rther
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SEkVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to:
