THIS CAUSE having come on for trial upon the pleadings filed herein
and all parties having received proper and timely notice; the Court having heard
testimony and/or considered the pleadings, papers, affidavits and other papers
filed herein,. and being otherwise Eully and well advised in the premises, it is
ORDERED AND ADJUDGED that ttie minor cliild(rer;
MARTIN NOEL GIBBON, d.o.b. 2-18-73
is/are declared to be the legitimate child(ren) of the Defendant
MAC AUTHER SMITH , and ANNA MAE GIBBON
the natural mother; it is further _
ORDERED AND ADNDGED that the natural mother,
ANNA MAE GIBBON , shall have custody of the said child(ren)
subject to the Defendant s.right of reasonable visits ion; it is further
ORDERED AND ADJUDGED that comaencing on C- ~ 1979,
the Defendant/Father shall pay child support for an n behalf of the said child(ren)
in the amount of c7J per ~Qe plus $2.00 statutory fee
All payment shall be made in cash, money order or cashiers check. All monev orders

Said amount shall be remitted monthty by the Clerk to the Department of t'.ealth and
Rehabilitative Services, Child Support Enforcement Unit, 13]7 Winewood Boulevard,
Tallahassee, Florida, 32304; it is further .
~ ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments from
the Department of Health and Rehabilitative Services, in which event-the support
payments shall thereafter be directed and payable to the aforesaid natural mother or
~ persotl-having custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to :4
