Court and by him remitted promptly to DEBRA JOYCE DAVIS. In addition,
there is hereby assessed the sum of $2.00 as statutory costs to be
added to each payment made to the Clerk. All payments shall be made
in cash, money order or cashiers check. Both of the parties are
ordered to keep the Clerk of this Court advised of their mailing and
residences addresses at all times while this Order is in effect.

4. MARITAL DEBTS. The Husband shall be required to pay
the following marital debts: Medical bill to Kane Clinic in the
approximate amount of $96.00; Medical bill to Radiology Associates
in the amount of $94.00; Medical bill to Lawnwood Medical Center in
the approximate amount of $33.75.
5. CHANGE OF NAME. The Wife's name is hereby changed from
DEBRA JOYCE DAVIS to DEBRA JOYCE JOHNSON.
6. MEDICAL INSURANCE. The Husband shall maintain a medical
insurance policy which provides coverage for the child of the parties,
which policy shall be with the Husband's employer.
7. ATTORNEY'S FEES. The-Husband is hereby ordered to pay
to the Wife's attorney the sum of $150.00 toward attorney's fees.
Said sum shall be paid on or before January 1, 1980. Said sum shall
be paid directly to the Wife's attorney, Charles J. Dorfman, Esquire,
