Before me, the undersigned authority, personally appeared
R.C. Minteer who, being duly sworn, says that he is the
agent of the lienor herein, LONE STAR FLORIDA, INC., whose address is
6451 North Federal Highway, Fort Lauderdale, Florida 33308 and that in
pursuance of a contract with Jerry Concannon
lienor furnished labor, services or material consisting of
Steel, concrete, etc.
of a total value of Four thousand four hundred twenty five and 27/100 dollars
4,425.27 ) of which there remains unpaid $ 4425.27
for which amount he claims a lien on the following described property in
Saint Lucie County, Florida:

He further says that lienor furnished the first of said labor,
