I HEREBY STATE:
That in the Book of Civil Registry
of DEATHS corresponding to the year
1973, in folio 394, it is found the
following, which true copy thereof states; "ARTURO ALFONSO LEVY LEVY"

Colombia, on the twenty fifth (25) of the month of August of one
thousand nine hundred seventy three, Jose Orlando Quevedo appeared
and stated that at S A.M. on the day of twenty three of the present
month, Mr. ARTURO ALFONSO LEVY LEVY died, being of male sex, and
being the age of 66, being from Faankfurt, Germany, being married,
and last occupation being a Salesman, that his death occured
at Cra 4a B. No. 32-43, that his legitimate parents were Daniel
Levy and Erieth Levy, that the cause of death was cardiac arrest, -

that same was certified by doctor Julio Velez Marquez.
