THIS CAUSE having come on for hearing on the petition of the DEPARTMENT
OF HEALTH Ai.'D tnH.~ILZTATIVE SERVICES for child support and the Court having found
that said Petitioner is a proper party by virtue of an assignsent of rights to
child support signed by the party having custody of the dependent child(ren) and
all parties having received proper notice, it is
ORDERED AND ADJUDGED that the Petition for Child Support shall be and
it is hereby granted; it is further
ORDERID AND ADJUDGED that commencing on JANUARY 18 1980,
the rcr~ponsible parent JOSEPH IDWARD CARR ~wl~o is the father ~
of the minor child(ren), to-wit: WAYNE CARR, d.o.b. 12-31-62; LISA CARR, d.o.b.
5-13-64; CAROL CARR, d.o.b. 10-10-66; ROGER CARR, .o. - - ,
d.o.b. 10-28-69• JOSEPH CARR, d.o.b. 11-25-70; KEVIN CARR, d.o.b. 9-28-73
shall pay to the Clerk of this Court the sum of $ 100.00 per month each and
every month , plus $2.00 statutory fee, until the AFDC arrears a of

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304. It is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and (s)he
is hereby ordered. to continue to transmit support payments received from the Respondent
to the Department of Health and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notice to Discontinue Paywents frog the Departseat
of Health and Rehabilitative Services, in which event the support payments shall there-
. after be directed and payable to the person having custody of the child(ren).
DONE AND ORDERED at Fort Pierce, St. Lucie County, Florida, on this
