THIS CAUSE having come on for trial upon the pleadings filed herein
and all parties having received proper and timely notice; the Court having heard
testimony and/or considered the pleadings, papers, affidavits and other papers
filed herein, and being otherwise fully and well advised in the preuises, it is
ORDERED AND ADJUDGED that the minor child(ren)
LaSHONDA DENISE PERRY, d.o.b. 10-11-74
is/are declared to be the legitimate child(ren) of the Defendant
ED KINDRICK - , and ANNIE ROSE PERRY ,
the natural mother; it is further `
ORDERID AND ADJUDGED that the natural mother,

ANNIE ROSE PERRY , shall have custody of the said child(ren)
subject to the Defendant s right of reasonable visitation; it is further

Said amount shall be remitted monthly by the Clerk to the Department of Health and j
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewnod Boulevard, t
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments frog
the Department of Health and Rehabilitative Services, in which event the support
~ payments shall thereafter be directed and payable to the aforesaid natural mother or
person -having custody of the child(ren) ; It is t::~:
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
x SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to:
