The Petitioner, ROGER JB'~iETT, alleges:
. 1. Petitioner has. an interest in the above estate as
nephew of the decedent, beneficiary-under the lost or destroy-
ed Wfll of Decedent and was appointed personal representative
under Decedent's lost or destroyed Nill. Pe-titioner's name
and address are: ROGER JEWETT, Box 94, Lake Jem, Florida
32745, and the-Wane and address of Petitioner's attorney are
set forth at the end of this Petition.
2. Decedent, BETTY J. JEWETT CUBBAGB, whose last known
address was Easter Manor Nursing Home, St. Lucie County,
Florida and whose age was 84 years and whose Social Security
Number is 276-10-7318 and died on September 3, 1979 at Easter
Manor Nursing Home and at the time of death Decedent was
domiciled in St. Lucie County, Florida.
' 3. So far as is known the names of the beneficiaries of
this estate, the names of all those who but for the Wili would
be entitled to the property thereby devised, their addresses.
