The undersigned, as guardian of the Estate of
of Lora Reaves - ,incompetent,:!leges:
1. The need for the continuation of this guardianship no longer e~tists by reason of-the fact
that Lora Reaves died on December 6. 147. i

2. Petitioner files herewith a final return containing a ful! and correct account of aU
receipts and disbursements of alt the- ward's property of which petitioner has control since the last
return filed herein, if any, or from the commencement of the administration of this guardianship if
no prior return has been filed, which return includes a statement of the ward's assets.
3. Petitioner represents that

WHEREFORE, petitioner requests that an order be entered approving aU accountings not
previously approved and discharging petitioner as guardian and releasing the surety on petitioner's
• -bond, if any, from further liability.
