Before me, the undersigned authority, personally appeared
A. A. DORSEY, who, being duly sworn, says that he is the President
of the lienor herein, DORSEY AND SON PLUMBING, INC. whose address is
2920 S. E. Kensington Street, Stuart, Florida, and that pursuant to a
contract with GIRARO AND LA ROCHE, INC. lienor furnished labor, services
and materials consisting of:
Plumbing Services and Materials
of a total value of X13,917.00 of which there remains unpaid X5,290.68
far which amount a lien is claimed on the following described real property
in St. Lucie County, Florida:
Tract "F" according to the plat of Port St. Lucie, Section
18 as recorded in Plat Book 13, page 17E of St. Lucie County,
Florida public records, containing 14.49 acres more or less.
owned by PORT ST. LUCIE, FLORIDA, a Florida municipal corporation.
Affiant further says that lienor furnished the first of said labor,
services•or materials on February 6, 1979 and the last of the same on December
13, 1979. Notice to Owner was furnished owner on March 2, 1979.
