BEFORE NE, the undersigned authority, personally appeared,
LEONARD SIMONS, who after being first duly sworn upon oath deposes and
says:
1. That he is the owner of the following described property, tcs wit: ~ .

Book 12, Page 13A of the Public Records of St. Lucie County,
Florida. 731 S.E. Albatross.
2. Lot 14, Block 400, PORT ST. LUCIE SECTION THREE, as recorded in
Plat Book 12, Page 13A, of the Public Records of St. Lucie County,
Florida a/k/a 762 S.E. Carnival Avenue.
3. Lot 19, Block 497, PORT ST. LUCIE SECTION TEN, a Subdivision
as recorded in Plat Book 12, Pages 49, 49A-49G, of the Public
Records of St. Lucie County, Florida a/k/a 1632 S.E. Pleasantview.
4. Lot 14, Block 496,_PORT ST. LUCIE SECTION TEN, a Subdivision
as recorded in Plat Book 12, Page 49A-G, of the Public Records
of St. Lucie County, Florida a/k/a 1682 S.E. Mistletoe.
5. Lots 15, Block 430, PORT ST. LUCIE SECTION THREE, a Subdivision
as on file in Plat Book 12, Page 13A of the Public Records of
St. Lucie County, Florida a/k/a 923 S.E. Albatross.
6. Lot 6, Block 496, PORT ST. LUCIE SECTION TEN, according to the
plat thereof, as recorded in Plat Book 12, pages 49 and 49A
through 49G, of the Public Records of St. Lucie County, Florida
a/k/a 1657 Floresta Drive.
2. That he had contracted with BURPHY CONSTRUCTION, INC.,' to be the

general contractor.for the construction of the home located on the above
described property; that said corporation is a registered corporation to
general contract and has performed all general contracting duties.
3. BURPHY CONSTRUCTION, INC., has sub-contracted portions of its
contract to the following sub-contractor, to-wit:

4. That all lienors who gave notice or filed liens to the undersigned
because of construction upon the subject property have been fully paid,
except those listed below who have not been paid:

office of the Clerk of the Circuit Court, and a copy of this Affidavit has
been served on each of the lienors, or possible lienors, listed above.
