property, acknowledges receipt of the assets shown distributed to the undersigned by said
accounting, acknowledges receipt of a copy of the petition for discharge of the guardian, waives all
notice and hearing on the petition for discharge, and consents to the entry of an order approving
the final accounting of the guardian and discharging the guaniian and the surety on the guardian's
bond, if any, without delay, notice, or hea~nr~g. _
