AFFIDAVIT

Before me, the undersigned authority, personally appeared
JOAN LAYCHAK HAWRELUK, who being by me first duly sworn, on oath
deposes and says:
1. That she presently resides at 12 Curt Terrace, Greenwich,
Connecticut; that she is a surviving daughter of JOSEPH CLARENCE
LAYCHAK, also known as JOSEPH C. LAYCHAK SR.~and JOSEPH LAYCHAK,
and hereafter referred to in this affidavit as JOSEPH CLA?tENCE
LAYCHAK; that she is a surviving step-daughter of ANNA ANITA LAYCHAK,
also known as ANNA A. LAYCHAK and ANITA LAYCHAK, and hereafter
referred to in this affidavit as ANNA ANITA LAYCHAK; and that she
makes this affidavit upon her personal knowledge.
2. That JOSEPH CLARENCE LAYCHAK and ANNA ANITA LAYCHAK
heretofore acquired and held title, as tenants by the entireties,
to the following described land, situate, lying and being in
St. Lucie County, Florida, to-wit:
Lots 22 and 23, Block 208, PORT ST. LUCIE SECTION 4,
a subdivision according to-the Plat thereof, ,
~ recorded in Plat Book 12, Page 14A of the
~ public Records of St. Lucie County, Florida;
by a warranty deed of_ conveyance, dated Senember 25, 1969, filed
f for record October I3, 1969, and recorded in Official Records Book
f 180 at Page 1181 of the Public Records of St. Lucie County, Florida. ,
3. That JOSEPH CLARENCE LAYCHAK and ANNA ANITA LAYCHAK were
married, each to the other, at Bunnell, Florida, on the 5th day
of April, 1952.
4. That JOSEPH CLARENCE LAYCHAK died a resident of Dade •
County, Florida, on April 7, 1977; and no probate administration.

5. That ANNA ANITA LAYCHAK died a resident of Dade County,
