THIS CAUSE having come on for hearing on Intervenor'• Petition
for Modification of final judgment or order on child support and the Court
having reviewed the file and considered the testimony of witnesses and being
otherwise fully advised in the premises, it is
ORDERID AND ADJUDGID that the Petition for Modification of final
judgment or order on child support shall be and the same is hereby granted;
it is further
ORDERED AND ADJUDGED that commencing on /~A N / ~ •
1980, the Father, RFN.iAMIN F. LUKE shall pay to the
Clerk of this Court the sum of $ 25.00 per week each and every
meek , plus $2.00 statutory fee,
which amount shall be rewitted sionthly by
the Clerk to the Department of Health and Rehabilitative Services, Child Support
Enforcement Unit, 1317 Winewood Boulevard, Tallahassee. Florida, 32304. All
payments shall be made in cash, money order or cashiers check. All money orders
and cashiers checks shall bear the payees name and Social Security dumber and shall
be made payable to the CLERK OF THE CIRCUIT COURT, and •ent to:

ORDERED AND ADJUDGED that in addition to the above-ordered aoounta the

hereby ordered to continue to transvit support payments received from the
responsible parent-to the Department of Health and Rehabilitative Services until
further order of this Court to the contrary or receipt of Notice to Discontinue
Payments from the Department of Health and Rehabilitative Services, in which
event the support payments shall. thereafter be directed and payable to the person
having custody of the child(ren).
