THIS CAUSE having come on for hearing on the petition of the DEPARTMENT
OF HEALTH AND REHABILITATIVE SERVICES for child support and the Court having found
that said Petitioner is a proper party by virtue of an assig~ent of rights to
child support signed by the party having custody of the dependent child(ren) and
al] parties having received proper notice, it is
ORDERED AND ADJUDGED that the Petition for Child S pport shall be and
it is hereby granted; it is further ~ /
ORDERID AND ADJUDGED that commencing on ~ ( 1980,
the responsible parent WILLIE H. DENNARD t who is the father

Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
TallahaesPe, Florida, 32304, It is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and (s)he
is hereby ordered to contim:e to transmit support payments received from the Respondent
to the Department of Health and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notice to Discontinue Paynenta from the Department
of Health and Rehabilitative Services, in which event the support payments shall there-
after be directed and payable to the person having custody of the child(ren).
