and all parties having received proper and timely notice; the Court having heard
testimony and/or considered the pleadings. papers, affidavits and other papers
filed herein, and being oth wise fully and well advised in the prea,ises, it is
ORDERED AND AD~ED that the migor child(ren)
CAROLYN,MARIE DAVIS, d.o.b. 9-5-70
is/are declared to be the legitimate child(ren) of tl,e Defendant
RAYFIELD YOUNG and MINNIE DAVIS ,
the natural mother; it is further
ORDERID AND ADJUDGED that the natural mother,
MINNIE DAMS , shall have custody of the said child(ren)
subject to the Defendant s right of reasonable visita~t/i~,on; it i further

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
~ Tallahassee, Florida, 32304; it is further .
ORDERED AND ADJUDGED that tt,e Clerk ,of the Circuit Court shall fnd is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments frog
tl,e Department of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural•s~other or
person-having custody of the child(ren); it is further
