THIS CAUSE having come on for trial upon the pleadings filed herein- •
and all parties having received proper and timely notice; the Court having heard
testimony and/or considered the pleadings, papers, affidavits and other papers
filed herein, and being otherwise fully and well advised in the premises, it is
ORDERED AND ADJUDGED that the minor child(ren)
PETER JUNIOR SMITH, d.o.b. 1-25-77
is/are declared to be the legitimate child(ren) of the Defendant
PETER SMITH , rind REBECCA HUDSON ,
the natural mother; it is further
ORD];RED AND ADJUDGED that the natural norther,
REBECCA HUDSON , shall Dave custody of the said child(ren)
subject to the Defendant s right of reasonable visitation; it is further
ORDERED AND ADJUDGED that commencing on /nA,~,y , 1980,
the Defendant/Father shall pay child s~~pport for and on behalf of the said child(ren)
in the amount of $15.00 per month , plus $2.00 statutory fee
until the AFDC is reimbursed in the amount of $1,100.00
All payment shall be made in cash. money order or cashiers check. All money orders
and cashiers checks shall bear file payees name and Social Security Number and shall

Said amount shall be remitted montt~Iy by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Miinewood Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that tl~e Cletk ~f the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from ttie Defendant
until further order of this Court or receipt of Notice to Discontinue Payments frog
tl~e Department of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural sorther or
person-having custody of .the child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been ad)udicated

SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to
