THIS CAUSE having come on for hearing on the petition of the DEPARI?~ENT
OF HEALTH AND REHABILITATIVE SERVICES for child support and the Court having found
that said Petitioner is a proper party by virtue of an assigcnaent of rights to
child support signed by the party leaving custody of the dependent child(ren) and
all parties having received proper notice, it is
ORDERED AND ADJUDGED that the Petition for Child Support shall be and
it is l~creby granted; it is further
ORDERED AND ADJUDGED that comeeencing on March 10 1980,
the responsible parent SAM HOOFER, JR. ~ wl~o la the father ,

BRENDA DAWN HOOFER, d.o.b. 10-28-78
shall pay to the Clerk of this Court the sum of $ 30.00 per week each and
every week plus $2.00 statutory fee,
All payments shall be made in cash.

payees name and Social Security Number and shall be made payable to the CLERK OF THE
CIRCUIT COURT, and sent to:

Said amount shall be remitted monthly by•the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304. It is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and (s)ht
is hereby c?rdered to contin~~e to transmit support payments received from the Respondent
to the Department of Health and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notice to Discontinue Payments from the Departssen[
of Health and Rehabilitative Services, in which event the support payments shall there-
after be directed and payable to the person having custody of the child(ren).
