THIS CAUSE having come on for hearing on the petition of the DEPARTMENT
OF HEALTH AND REHABILITATIVE SERVICES for child support and the Court having found
that said Petitioner is a proper party by virtue of an aseigntnent of rights to
child support signed by the party having custody of the dependent child(ren_) and
Sll parties having received proper notice, it is
ORDERED AND ADJUDGED that the Petition_for Child Support shall be and
it is hereby granted; it is further
ORDERED AND ADJUDGED that commencing on /11 tt r C ~ T, 1980,
the responsible parent JANNETTE SMITH ~ a?ho is the mother ~
of the minor child(ren), to-u?it: ISAMAN SMITH, d.o.b. 7-26-74; ISIS SMITH,
d.o.b. 4-4-76; CHARLES FLAGG, d.o.b. 5-2-77

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304. It is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and (s)he
is hereby .ordered to continue to transmit support payments received from the Respondent ;
to the Department of Health and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notice to Discontinue Payments frog the Department
of Health and Rehabilitative Services, in t~+hich event the support payments shall there-
after be directed and payable to the person having custody of the child(ren).
