THIS CAUSE having come on for. hearing on the petition of the DEPAlt1?lENT
OF HEALTH AND REHABILITATIVE SERVICES for child support and the Court having found
that said Petitioner is a proper party by virtue of an assignment of rights to
child support signed by the party Laving custody of the dependent child(ren) and
all parties having received proper notice, it is
ORDERED AND ADJUDGED that the Petition for Child Support shall be and

ORDERED AND ADJUDGED that commencing on 1980,

shall pay to the Clerk of this Court the sum of $ 35.00 per biweekl each and

payees name and Social Security Number and shall be ¦ade payable to the CLERIC OF THY
CIRCUIT COURT, and sent to:

Said amount shall be remitted sonthly by the Clerk to the Department of Uealth and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
I Tallahassee, Florida, 32304. It is further
ORDERED AND ADJUDGED that the Clerk-of the CircuiC Court shall and (s)he
is hereby ordered to contin~~e to transmit support payments received fro* the ><espoadeat;
to the Department of Health and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notice to Discontinue Payments from the Departs~eat
of Health and-Rehabilitative Services, in ~rhich event the support payments atoll there-
after be directed and payable to the person having custody of the child(ren).
